Consider neighbourhood when moving

I’m probably asking for something that doesn’t work this way in sculpting programs but when I use a soft selection brush in other programs I can set it to consider what is connected to the verts I’m moving.

For example if there is a gap between the top and bottom lip and I wanted to adjust the bottom lip I could still use a large brush without it moving the top lip even if the radius of the brush is large enough to reach the top lip. Hope that makes sense. The radius is based on that distance from the verts being moved.

I realise I can mask but hoping there is an option to do this, if not does this exist in full zbrush or does it just not work that way.

You can use the Move Topological brush, adjusting the Draw Size to control the influence.

Also, as an alternative, you can use the Transpose Line to create masking based on topology. Turn on Move mode, turn off the Gizmo so that the Transpose Line is active, and Ctrl+drag on your model.
