Doodled the idea on my sketchpad and finally found some time to work on it lately, face sculpted in Zbrush 3 and polypainted for basic texture/colour. Pipes etc modelled and rendered in Cinema 4D then some post work done in photoshop...

Nice image, was most of the work on the face in the final image done in photoshop.

Yeah I did most texture work in photoshop/c4d and rushed a little because I only had one night free to make it all so a lot was done in photoshop!!
The pipes were created in Cinema 4d, using a low poly head as reference and the highpoly head hidden but visible in the render. I created a load of parts/pipes and tweaked lots of points so they didn't intersect
